Sliding Fee Discount

If you only have Medicare, or no healthcare insurance at all, you may qualify for our Sliding Fee Scale (SFS) program. Eligibility is determined by the Federal Poverty Guidelines. The SFS could make the services we provide even more affordable. Please speak to a Health Navigator to see if you qualify (772-257-8224).


If you are pregnant and receiving Medicaid, please speak to one of our Health Navigators about “presumptive eligibility.”

Injured At Work?

If you are injured at work you must report the injury to your employer, get the appropriate insurance billing information from them, and provide to us.

Need Help?

We understand from time-to-time extraordinary situations occur that require additional consideration. If you find yourself in a challenging situation, we will work with you to review your payment and insurance options. You must communicate with us, don’t ignore us hoping everything will go away. Unfortunately, we must refer delinquent accounts to collection agencies. If you are unable to pay as agreed, please call one of our Billing Specialists at 772-257-8224.


YOU are ultimately responsible for the cost of your healthcare and all the payments. As a courtesy, we will process claims through your insurance provider. If the claim is denied, you will be responsible for the payment. Some payments are expected at the time of service. For example if you have a copayment, it must be paid at the time of your appointment.
