How do I make an appointment?

Call to schedule an appointment or request an appointment through the MyChart app. We have limited same-day appointments.

How much notice is required to cancel an appointment?

Many people in the community need affordable, accessible health care services. therefore, we insist on a 24-hour notice if a patient needs to cancel or reschedule an appointment.

What information should I bring to my appointment?

  • A valid form of photo identification
  • Insurance information
  • A list of current medications: prescribed and over the counter, vitamins and supplements.
  • Other important information such as glucose log, blood pressure record log, food log, etc.

What services are available at TCCH?

How do I pay for the services?

We accept a wide variety of private insurance plans, Medicare and Medicaid, and the uninsured.

The primary mission is access to quality healthcare for all. However, TCCH does not provider care at no cost. A variety of payment options and financial assistance is available for those who qualify.

Health Navigators can assist with self-pay, high deductibles payment options and a sliding fee discount based on income levels and family size.

Do I qualify for the sliding fee discount program?

TCCH offers a sliding fee discount program (SFDP) based on a patient’s ability to pay for services. The SFDP is established and implemented to ensure that uniform and reasonable fees and discounts are consistently and appropriately applied to all health center patients to address financial barriers to care. Eligibility for the SFDP will be based on income and family/household size. If you have additional questions, please call for an appointment with one of our health navigators.

All applicants need to provide the following documents at the time of the appointment:

  • One photo ID or two forms of verified identification
  • Verification of household income for the last 4 weeks
    • For working applicants, last 4 weeks checks stubs from employer or statements from employer indicating gross income.
    • For unemployed applicants, a statement from the applicant explaining how you have been living without any type of income. Letter of support if you are receiving help from anyone, they also need to provide a photo ID and a statement indicating they are helping you.
    • If receiving unemployment, copy of the check or a statement from the Unemployment Office.
    • For disabled applicants, a statement from the Disability office indication that you are disable and proof of you the amount of your disability check.
    • For social Security Applicants, proof of the amount of monthly check. The Social Security Office will provide a statement for you.

Can I choose my own doctor?

Yes! Let us know which doctor you would like to see when making an appointment. If you need immediate attention and your primary provider is not available, another provider will be happy to see you.

What if I prefer to speak a language other than English?

English, Spanish and Creole are the main languages spoken throughout the TCCH facilities. We also provide services for hearing and speech impaired patients along with 150 other languages through use of interpretation services and video conferencing options. These services allow TCCH to provide instant translations with a live operator.

Medicare Advantage offers extra benefits. However, original Medicare may be your best option. Explore your Medicare coverage choices.

If you make less than $22,000 a year ($30,000 for married couples), it may be worth applying for "Extra Help" with medication costs.
